Hypotension and hypovolemia during hemodialysis: is the usual suspect innocent?
Hypotension during intermittent hemodialysis is common, and has been attributed to acute volume shifts, shifts in osmolarity, electrolyte imbalance, temperature changes, altered vasoregulation, and sheer hypovolemia. Although hypovolemia may intuitively seem a likely cause for hypotension in intensive care patients, its role in the pathogenesis of intradialytic hypotension may be overestimated.

may be no blood-flow through the calf muscle of a claudicant during certain forms of exercise, in particular when the patient rhythmically raises himself up and down on tiptoe of one leg. It may also be true of an athlete that there is no bloodflow through his calf muscles when performing short and rapid sprints like running 100 yards in 10 seconds. My interest in this subject started when I fo...

Until the recent past, the search for the underlying drive for the pubertal increase in gonadotropin-releasing hormone (GnRH) hormone from the GnRH-containing neurons in the hypothalamus was largely focused on extrinsic factors. The most recent evidence however indicates changes in the structure of GnRH neurons themselves may contribute to this fundamental event in development. Based on our stu...

Frontline clinicians in the United States, especially those working in safety net hospitals or with immigrant populations, will likely see cutaneous tuberculosis given the tremendous burden of tuberculosis infection worldwide. The tuberculid is a subtype of cutaneous tuberculosis that poses a diagnostic challenge because organisms are not found in smears or cultures taken from the lesions. Tube...
متن کاملThe wrong suspect?
P atients with Parkinson's disease (PD) suffer a specifi c loss of dop-aminergic neurons from the mid-brain region that controls motor function. The exact mechanism of this selective neurodegeneration is unclear, though many lines of evidence point to dysfunctional mitochondrial complex I as one root cause of the disease. Yet Choi et al. now suggest that defective regulation of micro-tubules ma...
